Earlier this week, Tennessee’s first execution of a woman in modern history went terribly wrong when convicted murderer Christa Pike survived despite being given two lethal injections.
Now, the state’s prison chief has announced his resignation.
Tennessee Department of Correction Commissioner Frank Strada will be stepping down later this month.

Tennessee’s corrections chief is out after a botched lethal injection left convicted murderer Christa Pike hospitalized and on a ventilator.
Commissioner Frank Strada resigned Saturday as Gov. Bill Lee ordered an independent review into what went wrong. Pike was injected with two doses of pentobarbital Wednesday night, and prison staff used at least seven needles trying to find a vein. Her lawyers say staff failed to realize one of her veins burst, and Pike was heard crying, whimpering, and breathing loudly throughout the failed execution.
Strada said stepping aside was “in the best interest of Tennesseans.” Pike’s legal team says his departure isn’t enough, calling the incident a “systemic failure that goes far beyond any one person.”

Christa Pike, who brutally murdered a teenage girl in the 90s, remains critically ill and unconscious in the hospital since her botched execution three days ago.
Tennessee Gov. Bill Lee has launched a full investigation into the matter and paused all remaining executions for the rest of the year.
In his resignation announcement, Strada called the review
“entirely appropriate and necessary.”

Tennessee Department of Correction Commissioner Frank Strada, who oversaw the failed execution of Christa Pike, has offered his resignation and is stepping down later this month, Gov. Bill Lee announced in an emailed news release sent Oct. 3.
An interim commissioner will be appointed at a later date, Lee said.
Strada called serving as commissioner “a great honor” and that he continues to “believe the Department carried out its responsibilities in accordance with the established protocol.”
“Even so, stepping aside is in the best interest of Tennesseans as the independent review moves forward,” Strada said. “I believe this review is entirely appropriate and necessary, and I look forward to doing everything I can to help determine exactly what occurred.”
Lee ordered a third-party investigation into the failed Sept. 30 execution attempt, in which Pike was injected with two rounds of lethal injection drugs but survived.
“Commissioner Strada has served with integrity, and I appreciate his willingness to put the interests of Tennesseans first during this difficult moment,” Lee said. “I have accepted his decision to step down and am grateful for his commitment to ensuring a smooth leadership transition. I thank Commissioner Strada for his dedicated service to Tennessee and pray God’s blessings over him in the days ahead.”
Strada had been at the department since 2023. He came from Arizona’s corrections department, where he had served as a top official during three lethal injection executions that were later criticized for serious problems.
In light of his resignation, Strada’s record is being heavily scrutinized.
Prior to becoming the prison chief in Tennessee, Strada worked as Deputy Director of the Arizona Department of Correction, where he oversaw three more executions that had issues.

Pike’s attorneys, Randy Spivey, Kelly Gleason, Stephen Ferrell and Luke Ihnen, responded to the Strada stepping down in their own statement.
“The resignation of Commissioner Strada is justified, but it does nothing to help Christa now. What happened to her Wednesday reflects a systemic failure that goes far beyond any one person,” they wrote. “We hope the Governor’s call for a full, independent review will expose the profound problems within Tennessee’s entire death penalty system.”
Strada’s resignation comes after questions arose about his record as a correction official and other botched executions.
Strada began serving as the Deputy Director for the Arizona Department of Correction in January 2020 after serving as the director of operations for private prison operator The Geo Group, according to his LinkedIn page.
During his tenure in Arizona, he oversaw prisons where advocates say three executions were botched.
According to the Death Penalty Information Center, each of the three executions Arizona conducted in 2022 was “visibly problematic.”
Those three executions were the state’s first since 2014, when the lethal injection of Joseph Wood took nearly two hours and resulted in him visibly struggling to breathe.
After the three executions in 2022 went wrong, Arizona Gov. Katie Hobbs signed an executive order directing an independent review of the state’s execution policy and paused lethal injections in the state.
About a month before Hobbs signed her executive order, Tennessee’s Gov. Lee announced that Strada would take over the state’s Department of Correction.
At the time of his appointment, Tennessee too had paused executions after a 2022 incident when a lethal injection was halted at the last minute because officials failed to properly test the drugs.
By December 2024 — under Strada’s leadership — the Tennessee Department of Correction issued new protocols to resume lethal injections in the state.
It’s unclear what Christa Pike’s fate will be should she pull through in the hospital. Currently, she is being kept on a ventilator.
At this time, no second execution has been planned, but her death sentence has also not been overturned.
It’s likely that there will be a lot of legal challenges to come.
But, one this is for sure: this whole incident is raising a lot of questions about execution methods.
